Privacy Policy.

Active Rise Health (ABN 80655556264) is committed to protecting your personal and health information in accordance with the Australian Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs) and applicable state health-records legislation. This policy explains what we collect, how we use it, and your rights.

What we collect

We collect personal information you provide directly to us when you book an appointment, complete an enquiry form, attend a consultation or are referred to us. This may include: name, contact details, date of birth, Medicare number, health-fund and claim information (WorkCover, CTP, DVA, NDIS), GP details, medical history, presenting symptoms, treatment notes, imaging and outcomes data.

How we use it

We use your information to provide physiotherapy services, communicate with you about appointments and treatment, process claims with insurers and funding bodies, comply with our legal and professional obligations (including AHPRA record-keeping requirements), and improve the quality of our services.

Disclosure

We may disclose your information to: your GP and other treating practitioners with your consent; insurers and funding bodies (WorkCover, CTP insurers, NDIS, DVA, Medicare) where you have authorised a claim; the host medical centres at which we operate for shared-care purposes; and law enforcement or regulators where required by law. We do not sell personal information.

Storage and security

Records are stored in electronic clinical-records systems with access controls and encryption in transit. Paper records (if any) are stored securely. We retain clinical records for the periods required by NSW health-records legislation (generally 7 years for adults; until age 25 for minors).

Cookies and analytics

This website does not currently use third-party analytics or advertising trackers. Embedded Google Maps may set cookies governed by Google's own privacy policy. Contact and referral forms are processed by Formspree; submissions transit Formspree's servers.

Your rights

You have the right to access, correct, or request deletion of your personal information, and to make a complaint about how we have handled it. Requests should be made in writing to info@activerisehealth.com.au. If you are not satisfied with our response, you can complain to the Office of the Australian Information Commissioner (oaic.gov.au) or the NSW Information and Privacy Commission.
